Returned Good Authorization: PROCESS & PROCEDURE

All Returned Materials Authorization requests must follow the outlined process & contact procedure

A) Returned Materials Authorization numbers can be requested in a variety of ways. Please select from one of the below contact methods:

1) Online Requests can be submitted via the RMA Online Submission Form.

2) Please fax the Returned Materials Authorization form to (419) 869-1926

  • This form will need to be included in the returned goods shipping container with the authorization number

3) Contact the RMA Administrator

After request submission the RMA Administrator will contact you and issue the authorization number

Please note: No Return Goods Will Be Accepted Without a Returned Materials Authorization Number or Returned Materials Authorization Form

B) After the authorization number is assigned, please ship the goods to:

Attn: RGA Administrator: Reference: RMA ( insert authorization # )
